IMA Sleep is a fully accredited sleep center used for diagnosing sleep disorders. This facility is operated by physicians from Indiana Medical Associates. densitometry services to the area. This diagnostic test is used in the
detection of osteoporosis.
Northeast Indiana Endoscopy Center
The Gastroenterology physicians of Indiana Medical Associates, LLC own and operate a freestanding ambulatory surgical center. It is licensed by the State of Indiana and has accreditation with the Joint Commission of Accreditation of Healthcare Organizations. This Center specializes in Endoscopic procedures, aiding in diagnosis and treatment of various gastrointestinal conditions.

What to Bring on Your First Visit

Please Note:
Prior authorization of services is required by many insurance companies. Each policy is different. We must count on you to be familiar with your policy. Please read it over carefully. Your policy is a contract between you and your insurance company.
Unfortunately, failure to obtain the necessary prior authorization can result in nonpayment of fees by our insurance company. We don't want to see you in that position. So remember, check your policy and get the approvals first. We cannot be responsible for the amount your insurance company does not pay.
When You Arrive
Please "sign in" with our Receptionist when you arrive. If you are a new patient, one of the Registration staff members will talk with you briefly before your first visit with the physician.
Next you'll meet one of our Patient Escorts. The Escorts assist the nursing staff and physicians by weighing you, escorting you to and exam room, and taking your vital signs.
If you need a wheelchair, ask a Patient Escort for one. When you are finished with it, please return it to our waiting room. That way it will be available for another patient.
Registration/Cashier
Reviewing and entering your information into our computer system for billing and insurance purposes is the responsibility of the Registration and Cashier personnel. If there are any changes in your name, address, telephone number or insurance coverage please notify our office immediately.
Please bring your insurance cards with you to each visit. We will need to copy them.
Appointment Scheduling
Each appointment that you have with one of our physicians is set up by our staff in the Appointment Scheduling Department. The Schedulers will ask for telephone numbers where you can be reached should your appointment need to be changed. If you are unable to keep your scheduled appointment, please call at least 48 hours in advance to reschedule. This courtesy will give another patient the opportunity to be seen.

Note: Please arrive earlier that your scheduled appointment to allow for registration and updating of information. You may be asked to arrive early for lab work.
Medical Records
Information about you, your medical history and your case records are professionally maintained by our Medical Records Department. Be assured, your records will be treated as private and confidential. Please be aware that no member of our staff may speak with your family member or friend concerning your condition or care unless their name appears on the HIPAA authorization signed by you.
Upon request, the Medical Records staff will arrange to copy and forward to you, another physician or medical facility specific information when accompanied by a signed "Records Release Form" from the Medical Records Department.
The usual turnaround time for our copy service to send out a copy of records is 7 to 10 days. By Indiana law, the copy service is permitted to charge for this service and you may receive a statement for services from the copy service.
Please note, the information will be release only with the signed "Records Release Form." This form is to be signed by the patient, if 18 years of age or older. Spouses cannot sign for each other.

Insurance/Disability Claim Forms
Our company uses "HCFA 1500" Forms to file our claims with insurance companies. Most commercial insurance companies accepts this standard form. Please inform us if your insurance company has a special claim form that must be used.
Please mail or personally deliver Disability Claim Forms directly to our Disability Claims person. Be sure to fill out your portion of the Insurance/Disability Claim Form before giving it to us. This will help eliminate delays in processing.
In order to offer you the greatest protection, all information that we are asked to provide directly to an insurance company, attorney or social agency must be accompanied by a written authorization signed by you giving us permission to release the information.
